SGSUP Undergraduate Request Forms

The below forms are to be used to make official requests to SGSUP advising for the following issues:

  • Third Time attempts at coursework
  • Campus Change
  • Overload requests
  • Submission of required documents such as Probation Success Plans, Omnibus Course Requests, and Adjustments to Curriculum

All students who wish to request a major change must use the ASU Major Change Request Form. Please use the below button to formally request your major change to one of SGSUP's Programs:

  • Geography BA/BS
  • Urban Planning BSP
  • Geographic Information Sciences BS
  • Geography (Meteorology and Climatology) BS

  • Submit request for Urban Planning Minor HERE

If you would like to add a different minor or certificate, find the program HERE and request it through the minor/certificate page.

To remove a SGSUP minor or SGSUP certificate please email sgsup.advising@asu.edu with your request. If you are under a different major please contact your assigned advising team whose information can be found on your MyASU page.

Use the below form to submit your official request to enroll in an internship course. You must have an approval from the Internship Coordinator prior to submitting this request.

Use this form to make the following requests:

  • Request Permission to enroll in more than 18 credit hours for the Fall or Spring term
  • Request permission to enroll in more than 7 credit hours for either A or B-Session during the Summer term

You must complete the following form for any course override requests.

The College Course Override Request

Below are the requests that are permitted:

  • Prerequisite Override - You will need either permission from the instructor or documentation that the pre-requisite course is either complete or in-progress.
  • Class Full - You will need permission from your instructor and in many cases permission from the academic unit offering the course.  Even with instructor permission, an override may not be possible due to a lack of physical seats in the classroom and fire code restrictions. 
  • Instructor Permission - This override is for courses where instructor permission is necessary for enrolling in the course. 
  • Departmental Consent - This override is for courses where the academic unit must approve all enrollments in the course.
  • Time Conflict- This override is needed when your classes overlap or are less than 15 minutes apart.  If your courses do overlap, you will need permission from any instructors who are allowing you to miss class time in order to accommodate the time conflict.

The Third Time Enrollment Petition is the required document for attempting a course for a third time. Please review the below information thoroughly and use this form page to submit a completed document.

  • The petitioning process can take 1-2 weeks from date of submission during Fall or Spring semesters. Summer petitions may take longer.
  • Incomplete petitions will be returned without review.
  • The ASU General Catalog is the governing document for all policies and rules.
