THIRD TIME ENROLLMENT PETITION

Please follow the below instructions to ensure you're submitting the form properly:

  1. Download the fillable .PDF and save a copy to your PC
  2. Complete the fillable form and resave to your PC using an easily findable name (eg. Last Name - Adjustment to Curriculum)
  3. Complete this submission page and upload your recently saved form.

Petition Guidelines:

  • Petitions are for The College of Liberal Arts and Sciences majors only
  • The petitioning process can take 1-2 weeks from date of submission during Fall or Spring semesters. Summer petitions may take longer.
  • Incomplete petitions will be returned without review.
  • The ASU General Catalog is the governing document for all policies and rules.
